Forms

Please bring all forms that need to be filled out at the time of the office visit.

There is a 15.00 fee for the processing of forms unless it is done at the time of the visit. This fee offsets the time involved to pull the chart, review the information and get the form filled out. This includes camp forms, sports physical, FMLA, and all other forms requiring the patient's chart. If this is done over the phone, your account will be billed accordingly.
